• Name 1 — The first name line captured by your flow.
• Name 2 — The second name line captured by your flow.
• Name 3 — The third name line captured by your flow.
• Name 4 — The fourth name line captured by your flow.
• Card Number — The Medicare card number used for lookup.
• Card Expiry — The expiration date shown on the card.
• Card Type — The Medicare card type for the record.
• Individual Reference Number — The individual reference number used with the card record.
• Date of Birth — Used to match the correct card record.
