# Custom Form

> Empty verification page for the Custom Form product.

Custom Form lets you create and attach custom data-collection steps inside your verification flows.

## How it works

### Processes

The steps executed for this product depend on how you configure the form. When you choose a template or define fields manually, those configured fields determine what the flow captures and returns.

### Inputs

Inputs are the form fields you configure (either from a template or by adding them manually). The specific field list is determined by your configuration.

## Tool settings

Open **Tool settings** in the product settings modal.

Choose how you want to set up this form:

* **Use custom form template**
  * Select an existing template.
* **Create simple custom form**
  * Add fields manually.
